Form No. 158 - Creditors' Voluntary Winding-up Return of the final winding-up meetings of members and creditors

Creditors' voluntary winding-up: liquidator must file an authenticated return and annex meeting account and minutes to the Registrar.

Form No. 158 requires the liquidator to notify the Registrar that final meetings of members and creditors were held or summoned to consider an account showing how the winding-up was conducted and property disposed of, to attach that account and minutes, authenticate the account by the liquidator's signature(s), and indicate whether each meeting lacked a quorum.
