Affidavit formalities: personal declarations, signed and sworn, with strict exhibit marking and alteration requirements. Affidavits must be first person sworn statements stating the deponent's full name, age, occupation and place of abode, signed and sworn as prescribed by the Code or court practice. Exhibits must bear the proceeding number and be initialled and dated by the authority before whom sworn. Interlineations, alterations or erasures must be initialled by that authority, erased text rewritten in the margin and initialled, and filing of altered affidavits requires the Judge's leave.
