<h1>Affidavits must be in first person, signed, sworn, and any changes require approval or proper initialing.</h1> Every affidavit must be written in the first person, including the deponent's full name, age, occupation, and residence. It should be signed by the deponent and sworn according to the court's prescribed methods. Exhibits attached to affidavits must be marked with the relevant proceeding number and initialed and dated by the swearing authority. Affidavits with interlineations, alterations, or erasures require the judge's permission to be filed unless such changes are initialed by the authority, or, in the case of erasures, rewritten in the margin and initialed.
