List of creditors attendance template standardises recording of creditor names, proofs of debt, proxies and voting entitlement. Form No. 95 prescribes a template for recording attendance at a creditors' or contributories' meeting, requiring meeting venue and date, consecutive numbering of attendees, names of creditors or contributories present or represented, and the proof of debt or number of shares and votes held; attendance must be marked in person or by proxy, with a total of creditors or contributories present or represented and identification of the Official Liquidator.
