Petition for winding-up requires prescribed form submission in duplicate and an LLP's statement of affairs when filed by the LLP. A petition to wind up an LLP must be presented in the prescribed form (Form No. 26, 27 or 28) with any necessary variations, submitted in duplicate, and when presented by the LLP must be accompanied by the LLP's statement of affairs as at the date of petition; the Registrar or Tribunal must note the date of presentation on the petition.
