Affidavit Formalities require first-person deponent details, signing, swearing, exhibit marking and certification of alterations by sworn authority. Affidavits must be drafted in the first person and state the deponent's full name, age, occupation and place of abode; be signed and sworn or affirmed as prescribed; have exhibits marked with the proceeding number and initialed and dated by the swearing authority; and contain no uninitialed interlineations, alterations or erasures, with erasures remedied by rewriting the original words and initialing in the margin by the authority.
