The statement must be submitted in Form No. 37 in duplicate, with one copy verified by affidavit. An affidavit of concurrence, if applicable, should be in Form No. 38. The Liquidator is responsible for filing the verified statement and any affidavit of concurrence with the Tribunal promptly upon completion and retaining a duplicate for records.
