Prescribes that the application must be made in Form WIN 61, and mandates supporting material: where filed by the Company Liquidator, a signed statement of the facts forming the basis of the application; where filed by any other person, an affidavit stating the matters for which examination is sought and the grounds relied upon, thereby conditioning the court's consideration of the request on specified form and evidentiary support.
