Medical certification for confinement and miscarriage records pregnancy status, delivery, death before confinement, and authorised signatories. Medical certification is prescribed for confinement, miscarriage, medical termination of pregnancy, tubectomy operation, delivery of a child, and pregnancy-related illness. The form requires the certifying person to state whether the woman is pregnant, has undergone miscarriage or medical termination of pregnancy, has been delivered of a child, or died before, during, or after confinement, and to sign with designation and date.
