It required details such as the patient's name, address, the person on whom the patient is dependent, the disease or ailment, and the extent of any disability. The form also required the certifying specialist's details and verification of the patient's condition, including the extent of disability if applicable. The certificate needed to be countersigned by the head of a government hospital where the specialist practiced. This form has been omitted as per a notification in 2015.
