It allows a security holder to either cancel an existing nomination or appoint a new nominee for their securities, specifying details such as the nature and number of securities, folio and certificate numbers, and distinctive numbers. If a new nominee is appointed, their personal details, including name, date of birth, nationality, and relationship with the security holder, must be provided. In cases where the nominee is a minor, additional information about the guardian is required. The form must be signed by the security holder and witnessed.
