The form prescribes the information and documentary requirements for complaints to the Ombudsman: complainant and respondent particulars, prior representation and reminders, detailed grounds and evidence, copies of replies or rejections, the relief sought, a declaration of truthfulness and compliance with filing time limits and prior forum status, authorization to disclose materials to the Board/Ombudsman, and an optional nomination/authorization of a non legal representative whose actions bind the complainant.
