Duplicate licence affidavit to replace lost trade licences, requesting cancellation of original and undertaking return if traced. Affidavit requires declaration of loss/misplacement of licence/authorisation with licence number and date, the total amount issued and the balance for which a duplicate is sought, registration and utilisation status with Customs House, and affirmation that the original has not been cancelled, pledged, transferred, or handed over. Applicant must request cancellation of the original in favour of the duplicate and undertake to return the original if traced. Application must include payment evidence for the application fee and a copy of the First Information Report (FIR).
