Appeal application form requirement: appeals under section 35E(4) must be filed in Form E.A.-2 with supporting orders. An application to the Commissioner (Appeals) under sub-section (4) of section 35E must be made in Form No. E.A.-2, filed in duplicate and accompanied by a certified copy of the adjudicating authority's decision or order and a copy of the order directing that authority to apply to the Commissioner (Appeals).
