This form should be filed in duplicate and accompanied by two copies of the decision or order from the adjudicating authority, with at least one being a certified appeal copy. Additionally, a copy of the order from the Principal Commissioner of Customs or Commissioner of Customs, instructing the authority to apply to the Commissioner (Appeals), must be included.
