Form of appeal to Appellate Tribunal requires filing Form C.A.-5 in quadruplicate with certified decision copies. An appeal or application to the Appellate Tribunal must be made in Form C.A.-5, filed in quadruplicate and accompanied by an equal number of copies of the decision or order, one of which must be a certified copy, together with any required committee order.
