Applicants must enter the company's Corporate Identity Number (CIN) and use the 'Pre-fill' button to auto-populate the company's details. The director's identification number (DIN) and reasons for disqualification must be provided. A board resolution authorizing the signatory is required, and the form must be digitally signed by an authorized company officer. Optional attachments can be included for additional information.
