These reports must be submitted electronically in both PDF and Excel formats to a specified email address within 15 days after each quarter ends. The reports should be certified by an authorized official, and the email and file names should follow a specific naming convention. The previous format and terms from the 1994 circular remain unchanged, except for the reporting format and submission method.
