Form No. 138 notifies a named payee of a declared dividend, specifies payment amount and schedule at the Official Liquidator's office, and requires production of the notice with any negotiable instruments to claim payment. It provides enclosed Receipt and Authority forms for collection or to authorize delivery, notes signature rules for firms and companies, directs use of Form No. 139 to make payment payable to another person, and mandates remittance of unclaimed dividends after six months into the Companies Liquidation Account under section 555(1) of the Companies Act, 1956.
