....esulting into unwarranted delay in issuance of forms for no specific reason(s). To monitor the disposal of such applications the following guidelines are hereby once again issued as under:- 1. Applications for issuance of Forms in all Operational Units shall be received on all working days (except on Tuesday) up to 1.00 p.m. 2. As soon as the application is received a proper receipt shall be issued to the applicant and details of all applications received, shall be entered in a Form Issue Register to be maintained in the following format:- Sl.
