Format for Application of Self Sealing Permission for Export w.e.f. 01/09/2017 onwards

Have greetings for the day!

I have two queries as per below:

1. Export from the India by manufacturers and traders shall be executed on self sealing procedure based on Risk Management System from 01/09/2017 onwards as per Govt. notification.

My question is.. How to apply for self sealing permission at customs.? Is there any format available for making application.? If any, Please provide me in reply or tell the another way.

2. Govt. in notification has clearly mentioned to use Tamper Proof Electronic Seal to seal the container.. So, Please provide me exact specimen for the same seal. Is Tamper Proof Electronic Seal is the same as the One time Bottle Seal which Central Excise department have been using so far?
