EPCG License date problem

My Friend CA needs to authorize EPCG & it's procedures but in shipping bill filed by CHA for Exports we find only EPCG Licence number but not date & as per norms, please help us to solve this issue

Where can we find the date ?

Which date has to be mentioned in S/B for EPCG exports for fulfilling EO ?

As per rules it is mentioned...

'In case where shipping bill do not contain the EPCG authorization & date an affidavit from the authorization holder has been obtained that the export account for the fulfillment of specific EO against this EPCG authorization shall not be taken into consideration'

Please enlighten on this issue
