Affidavit formalities require first-person sworn statements with identity details, authenticated exhibits, and no uninitialed alterations.

Affidavits must be drawn in the first person and state the deponent's full name, age, occupation and place of abode; be signed and sworn in the manner prescribed by the Code or court practice. Exhibits must be marked with the proceeding number and initialed and dated by the attesting authority. Affidavits containing interlineations, alterations or erasures may not be filed unless such changes are initialed by the authority, and erased material is rewritten in the margin and initialed.
