It must be signed by the deponent and sworn according to the relevant legal code or court rules. Exhibits attached to affidavits must be marked with the proceeding's number and initialed and dated by the swearing authority. Affidavits with interlineations, alterations, or erasures cannot be filed unless such changes are initialed by the authority, with rewritten words on erasures also initialed in the margin.
