Appeals to the Appellate Tribunal must be filed in FORM HSNS APL-02 with the relevant documents, including the memorandum of cross-objections and the challenged decision or order. An immediate acknowledgement with appeal number is to be issued, and its date is treated as the filing date. If a self-certified copy of the decision or order is not submitted within seven days, the date of submission of that copy becomes the filing date.
