Appeals must be filed in FORM HSNS APL-01 with documents; acknowledgement with appeal number fixes the filing date. An appeal must be filed in FORM HSNS APL-01 with relevant documents, including grounds and the decision or order; the appellate authority must immediately issue an acknowledgement showing the appeal number, and the date of that acknowledgement is the date of filing, except where a self certified copy of the decision is not submitted within seven days, in which case the submission date of that copy is treated as the date of filing.
