We have lost original R Form (duly complete) in  transit pleas advice us how to claim for Refund Octroi.

Whether Refund claim is valid on Duolicate R FORM

First of all you would have to lodge a FIR for lost of document during transit. Submit an application to the Concerned Department stating the Fact and attach the FIR.  Get the payment made against octroi verified from treasury and request them to issue a Certificate in this regard.  On the basis of aforesaid documents,get the duplicate receipt or acknowledgement.

You may avail the refund on Duplicate R Form.
