Surrender of Duplicate PAN

The tin Webpage says, to surrender a duplicate pan, fill up the application for Change (or) Modification in PAN and fill up Coloumn 10 alone. But the facilitation centers are not taking up these applications and they say we need to approach Income Tax Department. Please advice the procedure.
