Form 3 - Application for cancellation or variation of nomination previously made in respect of savings certificates under section 6 of the government savings certificates, act, 1959

Savings certificate nominations can be cancelled or varied using Form 3 submitted to the post office with nominee and guardian

Holder must complete Form 3 under section 6 to cancel or vary a savings certificate nomination, submit it with the certificate to the Postmaster, name new nominee(s) (including date of birth for minors), appoint a person to receive proceeds if a nominee is a minor, provide certificate particulars, sign or thumb print the form, have witnesses attest, and obtain Postmaster acceptance by date stamp and signature.
