          <h1>Social Security Certificates application requires age 18-45, health and age evidence, Rs. 5,000 aggregate limit, and nomination.</h1> Form 1 prescribes the application procedure for purchase of Social Security Certificates under the Rules, requiring modes of tender, applicant particulars, a health declaration excluding specified diseases, and proof of age from an enumerated list of documents. Applicants must declare prior purchases or that aggregate face value does not exceed Rs. 5,000 and that no life insurance proposal was earlier rejected. The form provides for nomination (including guardians for minor nominees), optional identity slip with specimen signature, witness attestation, and post office administrative recording and transfer provisions.
