Voluntary Information Disclosure: Informants must file disclosure through counsel; Board may require originals to verify identity and accuracy. An Informant must submit the Voluntary Information Disclosure Form to the Office of Informant Protection, preferably through a legal representative; the Board may require personal appearance if not submitted by counsel and may demand re-submission in the prescribed manner or reject improperly filed information. The legal representative must verify identity and contact details, maintain confidentiality, certify completeness and truthfulness of the form, secure irrevocable consent to produce the original form when requested, and be legally obligated to provide the original within the period required by the Board. Informants must expunge or identify any content likely to reveal their identity.
