Document requirements for memorandum of appeal: file multiple copies, attach the challenged order and representative authorization. Rule 11 requires that every memorandum of appeal be filed in multiple copies and accompanied by copies of the order under challenge, at least one being a certified copy; when a party is represented, the representative's authorization and written consent must be appended to the appeal. The rule was amended to require five copies in place of the earlier triplicate requirement.
