The applicant must submit the prescribed application in Form A and pay the specified non refundable application fee in the manner set out in the schedules. Former transitional sub regulations addressing timelines for existing agencies and consequences of non filing have been amended or omitted, leaving the operative filing and fee requirement as the condition for commencing rating activity.
