An authorized officer must prepare this memo in the presence of the person from whom documents are seized, if available, and it must be signed by two witnesses. The memo includes a list of seized documents, details of the search location, identities of those present, and signatures of involved parties. For electronic media, a description of the storage media is required. The memo records entry and exit times, and identification marks, if any.
