Document Condition Notice: damaged or defaced exhibits must be indexed and verified upon production by authorized officer. When a document produced with a pleading is defaced, torn, damaged or otherwise requires special notice, the producing party must note that condition in the Index of the pleading; that notation and the statement about the document's condition must be verified and initialed by the officer authorized to receive the material.
