of <h1>Form No. 5: Memorandum of Appearance for Company Law Board Proceedings by Authorized Representatives.</h1> Form No. 5 under the Company Law Board Regulations, 1991, is a memorandum of appearance submitted to the Bench Officer of the Company Law Board. It is used by authorized individuals, such as a Secretary in Whole-time practice, Chartered Accountant, or Cost and Works Accountant, to officially enter an appearance on behalf of a petitioner, opposite party, Registrar, Regional Director, or Government in a specific company petition. The form requires a copy of the board resolution authorizing the representative to act in the proceedings, although this requirement can be struck out if not applicable.
