<h1>Petition Must Include Annexure III Documents and Index; Attach Form No. 5 or Vakalatnama as Applicable</h1> A petition under the Company Law Board Regulations, 1991, must include documents specified in Annexure III and an Index of Documents. These documents can be attested by the party, their authorized representative, or an advocate, and should be labeled sequentially as Annexures A-1, A-2, A-3, etc. If an authorized representative files the petition, a memorandum of appearance in Form No. 5 from Annexure II must be attached. If filed by an advocate, a duly executed Vakalatnama is required. Amendments in 1992 updated these requirements, effective from May 14, 1992.
