        Photo identity card authorization permits an employee to transact for a customs house agent while licence remains valid. Form G is a Photo Identity Card template for an authorized employee of a Customs House Agent, recording the employee's name, principal, customs station, validity period and confirmation that the employee passed the Regulation 19 examination.
