Proxy acceptance for incapacitated creditors: witness must add details and certify handwritten insertions before the mark is attached.

A blind or otherwise incapable creditor or contributory may attach a signature or mark to a proxy in the presence of a witness; the witness must add his description and address, ensure all insertions are in his handwriting, and certify at the foot of the proxy that those insertions were made by him at the request of and in the presence of the creditor or contributory before the signature or mark was attached.
