Company name change and availability requests are made using Form No. 01A under the Companies Rules. Form No. 01A is the prescribed application form under the Companies (Central Government's) General Rules and Forms, 1956 to apply for availability of a proposed company name or for a change of an existing company name; the form was later substituted by government notification.
